Reconciling national standards with local democracy will be a major issue. Part of the solution is to enable people to choose treatment outside their local areas. But if the people of a locality want convenient local health services and are prepared to pay the extra cost through local taxes, they should be able to do so. Local democracy is also needed to make managers and professional accountable to the people they serve.
