Community Care, Inc. (Community Care) is a privately held non for profit home care organization engaged in the provision of senior living services to seniors people and adults with disabilities in the Wisconsin region. The organization conducts community programs like PACE is a model which provides continuous care and services from the home to the hospital, family care partnership is same like PACE the only difference is that there is less dependence on adult day health centers; family care program and technical consulting services. It also provides services like religious orders, long term care, assisted living and retirement services, skilled nursing services, rehabilitation services, and behavioral health services. In addition, Community Care provides food services, personal care, medication supervision, and recreational activities, social and educational activities.
